To assist with the processing of your claim, send pictures of the damages that you have regarding this claim. Once we have received the claim form, it will be reviewed and the District or the insurance carrier will make the determination of liability. A claim for personal injury or damage to personal property must be presented within six (6) months of the cause of action.

If the actual amount of the claim is less than $10,000, indicate the exact amount of the claim; and if possible, show specific itemization and/or include copies of any documents in support thereof. If the amount of the claim exceeds $10,000, no dollar amount should be included in this claim form.

Claims against the District are governed by applicable provisions of the Government Code, and the District makes no representation that a claim will be accepted or approved. Persons with claims are encouraged to seek the advice of their own counsel.
